There are a lot of examples of IRB resources in academia (http://humansubjects.stanford.edu, http://research-ethics.net/topics/human-subjects/), but I don't know how much adapting they'd need to meet governmental review needs. But there seem to be these: http://www.hhs.gov/ohrp/index.html, http://www.epa.gov/oppfead1/guidance/human-test.htm (as for the last, 18F is probably not going to be involved in studies that might offer exposure to pesticides, but I've seen sociological studies that included "This study will not lead to pregnancy" checkboxes). Hope this helps?

For anyone watching / lurking: @andrewmaier and I had a good conversation about what research hygiene templates and scripts we might want to link to as "additional resources." I will bring the question about what resources we can make public and where we should put them to 18F's research guild. Stay tuned.

A link to 18F's consent form will be pushed to the online version of the methods cards on Monday. We will have a deeper discussion about the best place for other, similar resources to sit in our next research guild quarterly planning session.
