There are a lot of examples of IRB resources in academia (http://humansubjects.stanford.edu, http://research-ethics.net/topics/human-subjects/), but I don't know how much adapting they'd need to meet governmental review needs. But there seem to be these: http://www.hhs.gov/ohrp/index.html, http://www.epa.gov/oppfead1/guidance/human-test.htm (as for the last, 18F is probably not going to be involved in studies that might offer exposure to pesticides, but I've seen sociological studies that included "This study will not lead to pregnancy" checkboxes).
