How did you configure the plugin?

I think I found the problem, it isn't related to the plugin I run both Homebridge and PiHole in a docker and due to the nature of how containers work, they are not capable of communicating with each other so homebridge can't ping Pihole therefore the ip can't be reached that what's caussing the error.

Still need to learn how to make them communicate but that is another issue I hope to solve.

OK, I supposed that it wasn’t related to the plugin itself.

I do have both Pi-hole and Homebridge running on Docker, I configured Homebridge container with network set to host, as explained here, and Pi-hole container to map the admin port 80 to the 3000, in this way the plugin can reach Pi-hole using the host address.
