Figured it out. The default Sphinx style used a different even/odd page style in fancyhdr like you'd do for a printed book. The only Chapter-level heading in the main document was the heading for the TOC, which is why that was on even pages.

Adding the documentclass options oneside made it have uniform footers. Adding option openany I think removed the empty page after the TOC.

Note that the TOC and titlepage sections use a different style, so the header/footer might look funny there if they take multiple pages, and they still emit warnings during build because their style uses Odd/Even distinctions. #23
