I think the folks doing SQRL are pretty much state-of-the-art in terms of this sort of thing. Extremely easy and secure authentication, cross-device, without even needing to give the target site/app any of your data (of course, sending data is possible...). What they miss is the 1-account-per-person guarantee (quite the opposite).
Anyway, it might be worth looking at how they handle cross-device authentication.
