Hi, it is very rare to have 2 players kill eachother at the same time. CoDaM is quite big and bulky causing overhead making it even more rare for this happen, and the slower the host, the rarer it will be.
MiscMod doesn't do anything to limit players killing eachother that I know of. If instantkill is enabled and both hits are registered by the server, the chances should be higher that you get 2 players kill eachother.
