Update on example 2: the presence of a title results in a better (but still flawed) rendering.

Without title:

links [![my alt](/are.png)](http://foo.com) awesome

Renders as:

<p>
    links [
    <a href="/are.png)](http://foo.com">
        <img alt="my alt" src="/are.png)](http://foo.com">
        </img>
    </a>
    awesome
</p>  

With title:

links [![my alt](/are.png "my title")](http://foo.com) awesome

Renders as:

<p>
    links 
    <a href="http://foo.com">
        <a href="/are.png" title="my title">
            <img alt="my alt" src="/are.png" title="my title">
            </img>
        </a>
    </a>
    awesome
</p>

In the latter case, the orphaned [ is not present and the href and src attributes point to the correct URLs. So I think that the second bug isn't quite as severe as I originally thought.
