Here is a simplified testcase: 4326.zip

The issue occurs in both preprocess and preprocess2, inside KeyrefModule.

If I remove /id-fig from the keyref so it is a topic reference instead of a content element reference, the issue does not occur.

@Roopesh-Navath - are you sure this structure is what you want? You are creating two instances of the topic - one inside your keyscope, and one in the "default" keyscope. If this is unintentional, you might want to restructure your keys and keyscopes.

Publishing to PDF can hide these unintentional duplications. You could publish with --verbose and look for unexpected topic duplication messages. See this discussion for more information:

Yes. Large number of documents are having this scenarios. It is working fine in 3.5.4 and when I tried to upgrade the latest DITA OT, most of the links are broken.
