This one is interesting. And yes, there is a setting to avoid this in on the fly indexer configuration panel (use categories in url : No , defaulted to Yes).
I'll check since this should not happen in your case (root-catalog & default-category should have been skipped)

Another thing here.
I have for example category tree under Default Category:
Men/Shirts/T-shirts
Men/Coats
Women/...
Children/...
After import I go through normal Magento navigation Shirts or T-Shirts the url is respectively:
mywebsite.com/-/Shirts.html
mywebsite.com/-/Shirts/T-shirts.html
How come this "Men" is changed to "-"?
If I save the category Shirts in Magento admin panel and update URL Rewrite indexes the problem is solved and urls are:
mywebsite.com/Men/Shirts.html
mywebsite.com/Men/Shirts/T-shirts.html
