I think the issue is likely related to pooler not providing a means for workers to be shutdown cleanly and instead kills them with exit(Pid, kill).

I was just playing with the test suite code and I can repro an error like that if I hard kill a worker, but I'm not seeing it when I remove the pool. Not sure why yet.

I'd like to add a clean stop capability and I think that would solve this as well.

I believe #66 resolves this. It's still going to brutally kill the child in the same way but it won't generate a supervisor report since it's the supervisor itself who terminates the child.
