Thanks for the report.
What brand USB device are you using? Can you reproduce the issue with another USB device?
When I have time, I may implement a longer wait time for the USB driver (or a retry method).
Unfortunately I can't really tell when a USB device is available so the standard is to just wait for a few seconds.

The USB device is:
ID 0781:5567 SanDisk Corp. Cruzer Blade
I'm afraid I haven't got another spare USB to test, sorry. I did get the same outcome when trying to dump the BIOS on a (Phat) SCPH-39003 with the only difference being that I used FreeMcBoot to launch ULE. So it points to my USB drive being the likely culprit

I have since managed to get a BIOS dump (using the same USB drive) but it only worked when I copied the elf file off of the USB, onto a memory card and then executed it from the memory card. This method worked on both the (Slim) SCPH-7003 and (Phat) SCPH-39003.

The same thing is happening to me (PS2 Slim). I'm running FreeDVDBoot and can see "biosdrain.elf" in "mass:". "biosdrain.elf" runs, but I'm seeing the same message as zephyrj.

I don't have a memory card connected and am using the USB drive from https://www.bestbuy.com/site/pny-32gb-attache-4-usb-2-0-flash-drive-2-pack/6382188.p?skuId=6382188.
