I've also seen this, checked the source code and couldn't find any suspicious implementation in Files. As far as I can tell, this may be an issue with WinUI.

• Can you also confirm what your system theme is?
• Does the issue go away after restarting Windows?

• No, though I had to disable backdrop effects in Files' settings for the separator to be noticeable in light mode.
  

• I'm using Rectify 11 v3 along with a Windhawk mod to remove the 1px window border, however, even after disabling that mod and using any of the default Windows themes, the separator will still appear at random after opening the app window.

• Sometimes, yes. The tab separator seems to randomly show up despite there only being one tab even if its the first time opening the Files app after a reboot.

Can you replicate this in File Explorer or Notepad?
