Finding nr.1:
visual differences seems due to different font sizes set on root html node. On demo page it's not explicitly set and its value should be 16px at least on a test on Firefox. On our project page it's explicitly set to 10px (!) by https://maxcdn.bootstrapcdn.com/bootstrap/3.3.6/css/less/scaffolding.less. Not sure why this does not happen on demo page, as we could not figure out which Bootstrap version is used there.
Click behaviour is still broken anyway.

Finding nr.2:
Click behaviour was broken (also, or only, didn't check) because in some checkbox instances the label node was missing a for attribute matching with its sibling input. After setting that attribute, click works fine again.
Maybe it can be worth mentioning this change on readme, as it was not mandatory in earlier versions
