Hey @joseph-ortiz that error-id maps to "missing required attribute" exception. Looking at your code, I think the problem is that your component uses 'lightning:input' but doesn't provide value for a required attribute 'label' (feel free to consult component library's specification tab).
Kindly let me know if that helps.
