benefits:
- implementing permissions can be done easily
- force users to login to download
- prevent direct access to file via its link
- you can allow all kind of files (even php)
- this can be possible to save uploaded file with a random name and download with its original name using
Content-Dispositionheader

also Nginx, Lighttpd and Apache support xsendfile
Lighttpd has this feature and there is a mod_xsendfile for Apache2.
