I'm having same problems, but used same gmail account for both appspot and enketo. I
found another thread (https://groups.google.com/forum/#!msg/formhub-dev/qMamEHtuUbY/UXwEQkwiQ14J)that
mentioned something about a file named 'instances.sql' that was missing, but not sure
where to find it and what to do with it once found.

the Plan and Payment Tab inside https://accounts.enketo.org/account/.  If the 'Update
Plan and Card' button is clickable it is not confirmed. Simply click it and your issue
should be resolved.

the same issue. )

I faced a similar problem. Can confirm suggestion made in opendatakit/opendatakit#2 worked for me.
