I'm not sure I completely follow what you are looking for. This seems like two requests as well. Can you file the second request in a different ticket and expand it a bit?

Yes, two requests. This one should just be "don't change the case of the index values" ... I'll submit another one.

The intent here is to retain whatever string was entered, but group them case insensitively.
So it will be displayed as inputted, but 'MongoDB' and 'mongodb' would both match the same index key.

Does this work for you. I think the cases where you would want them to match the same key are pretty strong, but I could imagine not wanting this all the time. Help me understand a good use case for keeping them different and we could add a knob in the config that will let you do case sensitive indexes.

Imagine I'm using an index for menu creation. The menu item name should be as entered, the string used for links, etcetera, could be lower case with blanks removed

This is exactly the way I intended it to work (and how I've described it in
this ticket). If it doesn't this is an oversight and needs to be fixed.

See the recent commit to the docs site, particularly the menu.html file which does what you are looking to do quite well.
