BigQuery API -- List Row with 'start Index' and Big 'Max Results' induce wrong output.

@tswast @plamut Need your opinion as this issue is reproducible when passing startIndex >0 with page_size, In the response found same next_page_token every time so may be it's a backend issue, but it's working fine when startIndex=0 with page_size.

Temp Fix:
For now we can update startIndex manually here.

Don't use startIndex once you have a page token.

We should drop the start index from the request after the first page.
