io-pagination 找不到 common 的问题

迁移时没有去掉 common 导致的common/io-pagination-page-item未找到错误

io-form 设置readonly 的时候，出现calling set on destroyed object 错误

io-tree-select 设置 disabled 后，依然可以响应点击事件，并选择节点。

我安装成功之后，
<th>{{#io-button type="primary" }} 主按钮 {{/io-button}}</th> 放在一个组件里面。
控制台报错的内容为：Assertion Failed: A component or helper named "io-button" could not be found Error。

menu-item.js 的 mouseOver 问题

当触发二级菜单的mouseOver事件时，上级菜单会变成 io-menu-submenu-activeio-menu-submenu-open。多了io-menu-submenu-open

现在表格操作列里面引入模板太多，做成通用的模板，其他的做为参数传递

现在很多组件文档不够完善，需要逐个完善起来，方便开发者使用
文档目前直接写 handlebar，需要简化文档的编写步骤，直接使用 markdown 来生成，通过注释来生成 API 接口文档
