Philosophically - in most apps I have built does it matter? The app is starting up or the app is running, you still had a specific notification that you need to handle and in my experience when handling a specific notification it does not matter what the app state was, you still handle that notification in one deterministic way. So...doesn't matter?

Technically, any app I've worked with also has an initial startup sequence. That sequence has a beginning and an ending. So I will assume that your app either has that or you can add it. With that as a given, if prior to completing your startup sequence you see an event from Notifee, it's an initial notification.
