systemd socket activation is not really intended to be used with servers that don't implement the LISTEN_FDS protocol for handing over the listening sockets from systemd, and that feature is not currently implemented in ioquake3.
I suspect that the SO_REUSEADDR approach silently drops the packet that initiated the connection, and only succeeds because the Quake 3 protocol has application-level retransmission (which it has to have anyway, because UDP packets can be lost).
