I, as the creator of Dillinger, would love to be able to search all your files looking for the ".md" file extension without having to access all your folders.

Unfortunately, there is no way to do this.

However, you can certainly fork this repo and host it yourself and then only you will be requesting access to your dropbox, not Dillinger.io

I agree with the OP that it would be useful to be able to authorize dillinger.io to edit files in one specific folder, like http://fargo.io does it for example. Being able to edit all Markdown files in my Dropbox isn't necessarily a feature I need to have. (I have full understanding if this is not the way you want to go with your application of course.)

(Small sidenote/bug report, the options "Import from Dropbox" and "Save to Dropbox" currently behave in unexpected ways if there is no linked Dropbox account.)

Being able to edit all Markdown files in my Dropbox isn't necessarily a feature I need to have.

But it is for me! And for a number of other users. We have to search all of Dropbox's folders for MD files because there is not "file browser" of sorts in Dillinger.
