Hi,
I suspect klatexformula can't find ghostscript. (Which version of klatexformula are you running?) Check in (expanded mode) > settings > LaTeX > gs, and make sure that the path is correct. If the path is missing, find out where ghostscript is installed, and paste the path there (the path to the gswin32.exe file).
