I just sliced with Slic3r 0.5.7, default settings, and loaded the file in Pronterface. It seems fine:

Oops I should have said that it shows up fine there but when you go to print it, it starts off centered.

Then you have some issues with firmware configuration. Make sure your 0 is in the right position (corner of build plate) and that endstops work correctly.

Sorry for the long delay in responding, but if anyone else runs into this problem it is almost certainly due to a wimpy motor. I swapped it out with a high torque one (mine was from ultimachine but I am sure they sell them elsewhere) and my prints instantly started centering properly. Sweet!
