Have a look at app/controllers/admin/original_images_controller.rb line 5
This is not a dragonfly issue.
The reason it shows in your backtrace is that the middleware passes requests through to the main app.

Well line 5 simply says:

active_scaffold

that is why I believe the problem is the interaction between active_scaffold and dragonfly.
Since active_scaffold works fine everywhere else, I assume the problem comes from dragonfly.

Indeed the problem was not related to Dragonfly.

For future reference, the problem was caused by a model including

belongs_to :creator, :class_name => :User

should have been

belongs_to :creator, :class_name => "User"
