It is interesting to me that there are a couple of things that only work at the start of some construct:

• Frontmatter (extension) only works at the start of a document
• Definitions (CM) only work at the start of content blocks
• Task lists (GFM extension) only work at the start of list item phrasing

So it seems that there should be some hook for extensions to hook into
