Just an update:
I tried to change my files with the new commits done by other people's fork and also none of them worked. So after searching the web without finding anyone with the same problem I figured out it must be the OS. So I decided to try on a Linux distribution and it worked fine (without those other changes I had to do, to make the plugin work with windows)
I also tried to make a fix so that I could have the changes needed for the plugin to work in my windows and also the headers, but I've yet to find a solution.
It just doesn't work. For me, I'll have to keep using the plugin in a virtual machine with Ubuntu 10.10 and wkhtmltopdf-i386 until someone can find a solution (I'll keep trying too, but not for now)

I had the same issues on OSX. It was rendering the first time but repeated renders failed. I commented out
options[hf][:html].delete(:template)
Line 50 of pdf_helper.rb to force rendering on future calls
