I am having the exact same problem with both headers and footers. Nothing is rendered even though wicked seems to be doing just that in the console. Same specs as above except I'm on ruby-1.9.3-p0

Can anyone else confirm if this is a consistent issue on Mac OS?

This looks like it's setting information for wkhtmltopdf, are there issues with 0.9.9 on macos?

I was able to resolve this by uninstalling the wkhtmltopdf which I installed with homebrew. Manually installed from source and now it's working. This is on Mac OS 10.6.7. I have had no such issues with the apt-get installation on Ubuntu

I didn't install from source - but I did find a static version of later version (the google download and brew use 0.9.9) that resolves the issue.
