I think that that the host should be added in line 37 and line 39
As I looked in at the script a bit closer, in line 55 - shouldn't it be -
CREATE DATABASE $DBNAME OWNER $DBUSER;

Regarding the host, it probably makes sense to construct a single connection string (e.g. DBSTRING in https://github.com/MIT-LCP/mimic-code/blob/master/buildmimic/postgres/Makefile) that can subsequently be passed to each script.
Regarding the ownership, yes it was probably meant to be owned by $DBUSER.
