That seems to be an example of the expected case: the drug is started after it is ordered. Do you mean you've seen cases where the drug is ordered after it is started? That's still possible, as medications is via pharmacy. That is, in emergency situations, drugs may be started and then later on "ordered". @obadawi may have further insight into this and how frequent it may be.
EDIT: not the expected case above, but the unusual one with order occurring after drug start.

Thanks for the reply. In this, can it be assumed that started is same as administered ? Since it is mentioned that 'The medications table reflects the active medication orders for patients. These are orders but do not necessarily reflect administration to the patient'.

Since this information is usually sourced from an interaction with a pharmacy, you know that the drug was ordered and scheduled to start, but you can't say 100% that it was delivered. That being said the number of medications which are ordered but not delivered is by far the minority.
