This is almost certainly related to #2. (Firefox is using a different format for API experiments and the old format isn't supported anymore starting with version 61.)

I'm making an update ASAP.

If you want to use the theme right now you could use Firefox Developer (which is version 60 now).

I made a new release. You should now be able to use the addon in FF 61+.

Note that the installation instructions also changed a bit (it's now much easier, no need to install two separate extensions anymore).

On a side note, I never tested paxmod on Windows, so I'm really curious if it looks the same as in the demo gifs I made on Linux, or if you get a lot of design glitches. If you find the time I'd love some feedback on that!
