I'm so sorry for responding almost five months late. Github stopped e-mailing notifications to me, apparently. Since I assumed you get notifications when events related to your repo happen, I assumed no one had reported anything.

Now, on to your question (if this is still of use to you). You can include a single block in both the lexer and parser using the undocumented action scope, all:

@all::header {
  module MyModule
}
@all::footer {
  end
}

This should do the trick. If you still need this information, let me know if it fails or causes any weird problems for you. I was pretty careful to construct the code so that it can be contained in namespaces in this fashion.
