I just installed Xcode 5 DP4 and it seems to work fine for me, in Mountain Lion. Are you on Mavericks?
Things to try:
- Delete all traces of the plugin from your system and reinstall the plugin
- Try to add some NSLog's to the plugin code in order to determine where it's failing. You can watch for the output in Console.app
- I have no idea ... can you think of anything that might be different?

I'm on 10.8, with Xcode DP4. This is my first time installing the plugin. It built and installed itself in the correct folder, but opt+double click still loads the Xcode documentation viewer after multiple relaunches, etc.

Do you have a Dash Integration menu under Xcode's Edit menu?
Can you try to add some NSLog's to the plugin code to debug where things go wrong?

I do have the menu, and flipping the setting off then on again in the menu fixed it! Let me know if there's anything I can do to help figure this out; seems like a preference might not be getting set by default even though the menu item is checked on first launch?

What worked for me:
Delete the Plug-in
Reboot the computer
Rebuild/install the Dash plug-in
Quit Xcode
Start Xcode
Now I have the Dash integration menu back.

I switched from xCode 4 to 5 (final) and dash is gone. Doesnt even show up in edit menu , I tried all of above , but cannot make it work :( Any ideas? Thanks

Try reinstalling the plugin. @omz just pushed a commit that adds support for the Xcode 5 GM.
