Disabling auto-reload causes confusion/complication with the menu because the menu then has to indicate the current state after changes are made but before the next reload.

Ultimately, a better solution is probably the one in #2.

Fixed in r306. Just added an option to Preferences > Advanced to disable auto-reload.

When auto-reload is disabled, the menu doesn't automatically update in any way when changes are made. So, it isn't a great solution. If there are enough users of this and they are very annoyed by it, I'll decide if an improvement will be easier than just implementing #2 (which probably won't be trivial).

I'd really prefer the behavior of [https://www.requestpolicy.com/dev/ticket/2 #2] over this, but if it's too much work, theres more important things than this.
