.htaccess and other server files as samples for URL Rewriting

Added 2 samples, a basic index.php and a README to the new public folder.

We should probably consider other web servers too?

• index.php - a starter and most basic Respect/Rest application.
• basic.htaccess - the most basic Apache rewrite rule to direct all requests to index.php (rename to .htaccess to use)
• no-slash.htaccess - in addition to using index.php for requests also remove trailing slashes / (rename to .htaccess to use) The file also contains the rewrite rule should you always want a trailing slash instead, see comments.

I'm starting to think that this should probably be on Foundation as well.
