Adding documents to node-sources does not prevent duplicated in database

Here is the magic SQL query to remove duplicated, always perform a SQL backup before:

delete from nodes_sources_documents 
where id not in (
    select t.id from (
        select max(id) as id from nodes_sources_documents group by ns_id, document_id, node_type_field_id
    ) as t
);
