Smarter Feescales

when a treatment is added, currently only one feescale (the best match for patients age/contract etc) is searched. If not found, then "other treatment" is returned.
I now intend to search all active feescales for the treatment code. An obvious example of this is automatic allowance of private only treatments (post white fillings/ implants/ flexidentures) can be added to an NHS course in a less manual way.

add completion functionality to the listViews adjacent to the chart widgets.

treatments added as "FST" not handled well by recalc estimate, and ambiguous in the notes when completed.

om_pyuic4.py depends on bzr, move to git

this script, my version of pyuic4, for converting qt-designer ui files but using gettext rather than qt translation tools, currently assumes the ui files are in a bzr tree.

Estimate sign-offs.

This is a security enhancement. To protect the practice and the patient, it is essential that estimates are checked by the clinician before a record is saved.
Therefore a new mode will be added such that if an estimate is altered during the period the client application has a record open, those changes can only be committed after explicit user authenticated. With every alteration, a text representation is stored to the database, in a new table est_history.
