Seems like it might be worthwhile exploring wal2json again: https://github.com/eulerto/wal2json

This is supported by PostgreSQL 9.4+ and RDS 9.6+ (I believe)

Also worth noting that for some reason wal2json doesn't appear in the extension list of PG

The key change will be somehow passing wal2json as the output plugin when connecting epgsql and creating the replication slot, which seems to be achievable here: epgsql/epgsql#184

Resolution was to send all the data (columns etc) back to the client. Will leave it to the client libraries to do the mappings
