After some more investigation it looks like the --fix for this rule only works if it's the first line (as that's what is being tested).
I made a fork, with a test case proving the bug, and will be submitting a pull request.
It looks like everything was offset by the way the source code was formatted. The code that was there to grab it as a string was stripping any leading newlines and whitespace, putting the token start/end character counts off by whatever amount.
