Please update to version 1.5.1 which came out yesterday and adds support for function breakpoints. There is a bug in VS Code Feb release that causes the old versions to break.
Also, please note that the source root settings are ignored when program is used (second launch config) because the script is always launched on the local machine.
Lastly, there is definitely communication going on between XDebug and the adapter, so you should check the permissions on your /var/log/xdebug/xdebug.log. It must be writable and the folder executable by your web server user (probably www-data).
