You should not have to rename any folders. Each folder represents a module you can access through the URL (such as "example/index"). If you built a "/forum" module then it would look like this:

/forum
    /controller
        /index.php
        /create.php
    /view
        /index.php
        /create.php

Also, support for Apache has been dropped since the idea of a slow server does not make sense with a fast framework. Nginx is the recommended server. On windows you can use WNMP and on debian you can run sudo apt-get update nginx.

The .htaccess was left there as a curtsy to those which still require apache. However, I am not sure if the file is still accurate and according to your 500 error, I would say it is broken. I would recommend you replace that .htaccess with the following simplified version:
