Do not show info page at start

Instead of info page we should show TOC at start.

Basic template for different mobile interface

Our current mobile interface is not that perfect on other devices. Tried it on Kindle Fire and Android telephone.

Try something new where header/footer would be visible when you touch the screen.

Use DOMParser to parse external HTML files

At the moment we are using jQuery to manage external document. What we should be doing is fetch external data as pure text, parse it with DOMParser and manage it later on.

The problem with jQuery is it tries to load external references as you load the document. This should not happen.

Different functionality for internal and external links

For now just have show internal links in the reader, ignore links to other internal references and show external links in new window.
