USSD Getting Started is Missing in README

I have setup the sdk properly but I do not know how I can initiate the code to respond to my USSD requests. Can this be added in the README? I am kind of stuck. Or if there is anyone who can help with how I can do that.

Document on how to get started contributing to the gem

First thing that comes to mind is how to setup the environment variables that are introduced in PR #2

Strict HTTParty version

The gem currently has a hard HTTParty version of 0.16.4 in the gemspec which limits its use in other projects that might be using a higher version. Suggest making the version ~> 0.16.4
