I tested with Firefox 94.0.2 on macOS 12.0.1 with your settings and it worked fine on http://localhost:8080/examples/index.html

Safari Tech Preview 135 (Safari 15.4, WebKit 17613.1.7.3) also worked fine.

I didn't work properly with latest stable Safari 15.1 (17612.2.9.1.20) desktop.

Haven't tested with iOS.

I updated Firefox and tested with Firefox 96.0.2 again. It worked fine!
In my environment, in the case which prohibits autoplay, default volume is muted. So, I think I misunderstood it can't play.
I'm sorry.

And it worked fine on future version of safari is good news!
