Pin queue dispatch to promise job queue

The reference implementation has this really cool comment explaining why they don't solely use process.nextTick and instead call it at the end of a resolved promise.

From the comment it sounds like a safer way to wait to run the queue dispatch function until any promise actions in the promise job queue for the current frame of execution having been handled.

It'd be cool to write a test for this too.

Add promise cache

Caching a promise for each load key sounds like a pretty cool thing to do.

The reference implementation includes a default cache and special caching options. Having just a default one for now seems like a good start.
