Switch ItemList from ArrayAdapter to SimpleCursorAdapter

This will scale better without reading the whole database into memory (not that anyone has that many passwords). Similarly, change the search filter from filtering the ArrayList to performing a filtered database search, I guess.

Prepend exported databases with metadata

Importing an exported database means assuming the same cipher settings and KDF iterations as the defaults. This means import/export/backup between different app versions will break if we change those settings.

Suggested fix:

1. On export, copy the database to a file consisting of [[metadata]][[database]]. Metadata could be a protobuffer for forward/backward compatibility, but that may be overkill. On import, read the metadata and set the proper cipher/key settings.
2. On backup, either do the same or also backup a Preference of the same data.
