Markov node after decision node

Think of a way to embed Markov nodes/ models into a decision tree.

Two ways:
Decision tree followed by either probabilities all the way to the end to terminal nodes, or by Markov nodes/ models

Create an OpenTree function that calculates expected values

Seems like there should be internal functionality to calculate the expected value of each payoff for each strategy (e.g., not just the list with path probabilities and path payoffs). This wouldn't be an involved function, but seems weird we don't have it!

Fix hexsticker

Make hexsticker brighter and the text more visible
The tree could be brighter too

Payoff box

We could reformat the pay off box to have single vs. two payoffs.
If it's too much work, we could add text indicating how to properly insert the payoffs. For example: cost; QALYs
