Name, Email, File

I need a form that has 3 input fields like this:

name1,
email1,
file1

name2,
email2,
file2

etc..

And then display the data in a list:
name1 - email1 - http://file1
name2 - email2 - http://file2
etc..

I am able to do both things separately, but i can not figure out how to attach the file to the user id.
Should it be 1 collection that holds the name, email, file or 2 separate collections and link them together by the id? Please help!

Using with quickform?

I tried adding it to my quickform:

CvSchema = new SimpleSchema({
    name:{
        type: String,
        label: "Nume",
    },
    email:{
        type: String,
        label: "Email",
        optional: true,
    },
    images: {
        type: [String],
        optional: true,
        autoform: {
            type: 'ufs',
            collection: 'images',
            store: 'ImageStore',
            publication: 'images',
            //thumbnails: 'thumbnails'
        }
    },

Now the form looks like it knows about it, but it doesn't work and it throws error:

Deos it work with quickform or do i have to change it to autoform?
