Refactor Kindle plugin so it is a customization of epub plugin

The Kindle plugin essentially produces epub files and then runs kindlegen on them. Because the code was nearly identical to the epub2 code, this requires updating code (e.g., bug fixes) in two places. Better approach is for Kindle plugin to execute the epub plugin with XSLT overrides to allow for taking advantage of Kindle-specific features and then run kindlegen on the generated epub (or if preferred, the files in the epub archive, although Kindlegen takes an epub file as input).
