Fix the encoding problems when outputting JavaScript and HTML from PHP

JavaScript and some HTML is not encoded completely correct (e.g. umlauts) when outputted by PHP.
The problem seems to be more severe when using a Browser other than IE (tested with version 11) and Apache as the webserver.

Convert the documentation to markdown

The documentation (/docs) currently exists in the .docx (Microsoft Word) and .pfd formats.
A conversion to markdown would be beneficial due to multiple reasons, compatibility being one of them.

Fix the php warnings when using Apache as the webserver

The application was originally developed and tested for the use with an IIS webserver.
Due to differences in handling some code parts, php warnings will be shown if they are not turned off when using Apache. This is an annoyance, doesn't affect the functionality though.
