Consolidate repetitive code in index_maker()

Need to DRY it; Basically this should all be a function..

if interactive is True:
+                custom_label = custom_labeler(format_var[1])
+                format_var[0] = custom_label
             write_to_file_string = "[{0}]({1}){2}".\
                 format(format_var[0], format_var[1], link_ending)

Allow other extensions

$ indexmaker --extensions py, md, html

Generate links for various file extensions

Add license

Allow two formatting options

For output links, allow:

• double space (single line break)
• \n\n ending (double linke break)

Handle existing index.md

Rename index.md to index_old.md

Add interactive mode for choosing hyperlink name

ie, generated was:

[example_dir2/dir2_subdir/subdir2.md](example_dir2/dir2_subdir/subdir2.md)

allow user to choose (interactively)

[the subdir2 thing](example_dir2/dir2_subdir/subdir2.md)

Find .markdown files, too

Currently only looking for .md files
